Thank you for your post. We are aware of the M&T Bank is updating for most customers but it is showing a 10% failure rate, which is higher than normal. We are aware of the connectivity issues that prevents your M&T Bank account from adding and refreshing in Mint.
The error returning requires you to setup your security challenge questions in the bank's website and once completed, go back to MINT and try to update your login details to update/ add your accounts.
For reference, please follow the steps below:
1. Log into your M&T Online Banking Account
2. Click "Settings and Support" > "My Profile"
3. Click "Edit" next to Security Questions
4. Sign into Mint and try again.
